Make sure you choose the correct packet: 

Adult packets are for Adults and Couple's only*

Child packets are for persons 17 and younger only*

 

Please print and fill out the packet and bring it with you to your first appointment. 

 

For Kaiser patients, please include your Kaiser Authorizaiton Number on the intake form*
